Using the form:
Choose Size, Color of Logo, and backing. If you want your text a color other than black, use the additional info box.
Also use the additional info box for any set up requests that differ from the sample design or to let us know exactly how you want your tag set up.

Slotted name tags are for lanyards so they do not need backings, so just choose pin back (but note, one will not be sent unless requested in the additional info box)
